{"id":3771,"date":"2020-07-21T04:29:05","date_gmt":"2020-07-21T04:29:05","guid":{"rendered":"http:\/\/orionhealth-blog.local\/?p=3771"},"modified":"2024-08-12T23:49:24","modified_gmt":"2024-08-12T23:49:24","slug":"what-does-fhir-version-4-entail","status":"publish","type":"post","link":"http:\/\/orionhealth.com\/global\/blog\/what-does-fhir-version-4-entail\/","title":{"rendered":"What&#8217;s the big deal with FHIR 4?"},"content":{"rendered":"\n<p>It\u2019s been almost a year since HL7 announced their&nbsp;latest&nbsp;version of FHIR, version 4.&nbsp;Many healthcare industry players have been eagerly awaiting the fourth iteration of FHIR.<strong>&nbsp;<\/strong>As it continues to evolve, the FHIR standard&nbsp;will be backwards compatible from now onwards. Many foundational&nbsp;artefacts&nbsp;of FHIR 4&nbsp;are now defined as \u201cnormative\u201d, meaning,&nbsp;they will go through a&nbsp;standard development lifecycle before they can be modified.&nbsp;&nbsp;&nbsp;<\/p>\n\n\n\n<p>As&nbsp;the latest version of FHIR, anyone involved in the sharing or consuming of healthcare information will need to pay&nbsp;close attention to version 4 for all it has to offer.&nbsp;&nbsp;<\/p>\n\n\n\n<h4 class=\"wp-block-heading\"><strong>What does FHIR version 4 offer?<\/strong><\/h4>\n\n\n\n<p><strong>FHIR community resources<\/strong><\/p>\n\n\n\n<p>FHIR is now a global healthcare standard,&nbsp;indicated by many official HIT bodies and organisations as the means to achieve interoperability.&nbsp;<\/p>\n\n\n\n<p>For&nbsp;instance, the Office of the National Coordinator for Health Information Technology (ONC)&nbsp;in the US&nbsp;and&nbsp;the National Health Service X (NHSx)&nbsp;in&nbsp;the UK,&nbsp;are adopting the FHIR standard&nbsp;to coordinate nationwide&nbsp;data-sharing&nbsp;programmes.&nbsp;The version they rely on is&nbsp;FHIR R4&nbsp;v 4.0.1.<\/p>\n\n\n\n<p>Due to&nbsp;its&nbsp;global prevalence, the FHIR community provides tooling, training and resources available with country-specific content.&nbsp;HL7 encourages and supports all adopters of FHIR, from established healthcare networks to&nbsp;entities just&nbsp;starting&nbsp;on&nbsp;their interoperability journey.&nbsp;<\/p>\n\n\n\n<p><strong>Updated implementation guides<\/strong>&nbsp;<\/p>\n\n\n\n<p>The US Core Implementation Guide is based on\u00a0<a href=\"http:\/\/hl7.org\/fhir\/R4\/index.html\" target=\"_blank\" rel=\"noreferrer noopener nofollow\">FHIR Version R4<\/a>\u00a0and defines the minimum conformance requirements for accessing patient data. We see an increasing number of\u00a0<a href=\"http:\/\/www.fhir.org\/guides\/registry\" target=\"_blank\" rel=\"noreferrer noopener nofollow\">Implementation Guides<\/a>\u00a0that utilise FHIR 4 as their base. These new guidelines\u00a0can be set\u00a0to\u00a0represent the\u00a0needs of a specific country, with\u00a0the\u00a0related\u00a0variations in the structure of\u00a0FHIR\u00a0resources (e.g. the <a href=\"https:\/\/www.hl7.org\/fhir\/us\/core\/\" target=\"_blank\" rel=\"noreferrer noopener nofollow\">US Core Implementation Guide<\/a>)\u00a0or\u00a0to define\u00a0rules of integration to satisfy specific\u00a0use cases in an open ecosystem. (e.g.\u00a0<a href=\"http:\/\/www.hl7.org\/fhir\/smart-app-launch\/\" target=\"_blank\" rel=\"noreferrer noopener nofollow\">SMART App Launch Framework<\/a>).\u00a0<\/p>\n\n\n\n<p><strong>Increased resource maturity<\/strong>&nbsp;<\/p>\n\n\n\n<p>Version 4 boasts a vast number of changes&nbsp;regarding&nbsp;the&nbsp;detail of the individual resources that FHIR defines and&nbsp;new resources needed&nbsp;as FHIR&nbsp;continues to extend.&nbsp;&nbsp;<\/p>\n\n\n\n<p>The FHIR standard has now reached a high level of maturity. This means any changes to &#8220;normative&#8221; aspects of FHIR resources will be&nbsp;backward&nbsp;compatible. All resources have a \u201cmaturity\u201d level associated with them, which indicates how well they have been exercised in the real world. The&nbsp;higher the number, the more mature they are and the less&nbsp;likely&nbsp;to&nbsp;change&nbsp;between FHIR releases.&nbsp;&nbsp;<\/p>\n\n\n\n<p><strong>Related specifications<\/strong>&nbsp;<\/p>\n\n\n\n<p>FHIR 4&nbsp;features associated standards and projects that build on the FHIR Core to address specific implementation issues. The&nbsp;CDSHooks,&nbsp;DaVinci project&nbsp;and&nbsp;FHIR Bulk Data Access APIs&nbsp;are&nbsp;some&nbsp;examples.&nbsp;These projects embraced the FHIR standard and added value that goes beyond the enablement of a data-sharing environment. They respectively leverage decision support, payers&#8217; workflows and population management, through the standard clinical resources provided by FHIR.<\/p>\n\n\n\n<h4 class=\"wp-block-heading\"><strong>How is Orion Health involved in FHIR 4?&nbsp;&nbsp;<\/strong><\/h4>\n\n\n\n<p>Orion Health&nbsp;has been working with FHIR APIs for&nbsp;a number of years now, since the early versions&nbsp;were released.&nbsp;Keeping up to date with the evolving FHIR standard, Orion Health is currently&nbsp;building&nbsp;FHIR R4 APIs.
