Clinicians and care teams can now access up-to-date, patient-specific care preferences in real time, improving quality, consistency, and communication at life\u2019s most critical moments.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\">A New Era of Citizen Empowerment: NHS App Integration<\/h3>\n\n\n\n<p>By connecting the DCCR to the NHS App, patients will soon be able to view outpatient appointments, care plans, and other critical information through a single, secure interface.
