Alberta CII  aims to improve continuity of care for Albertans through better sharing of patient information. It does so by allowing data collected in community-based primary care clinics available to more provincial healthcare providers through Netcare (Alberta’s Electronic Health Record). CII provides a more complete longitudinal patient record (that is stored in Netcare) by including the primary care information The information will also serve as a key asset in health analytics for quality improvement, population health assessment and health system planning. This program required a collaborative approach that engaged both the community and primary health care partners.
