{"id":2579,"date":"2020-01-14T01:56:24","date_gmt":"2020-01-14T01:56:24","guid":{"rendered":"http:\/\/orionhealth-blog.local\/?p=2579"},"modified":"2024-06-10T02:53:00","modified_gmt":"2024-06-10T02:53:00","slug":"the-shift-from-reactive-to-proactive-care","status":"publish","type":"post","link":"https:\/\/orionhealth.com\/global\/blog\/the-shift-from-reactive-to-proactive-care\/","title":{"rendered":"The shift from reactive to proactive care"},"content":{"rendered":"\n<h4 class=\"wp-block-heading\">A health system designed for Acute Disease&nbsp;<\/h4>\n\n\n\n<p>Globally, most health systems were designed in the post-World War&nbsp;II&nbsp;era as the \u201csick care\u201d system.&nbsp;For the most part, the system was not designed to help prevent the onset of&nbsp;disease but instead to diagnose and treat illness.&nbsp;&nbsp;<\/p>\n\n\n\n<p>Acute diseases&nbsp;tend to be short, diagnosed easily and treated with a cure. The reactive approach to acute diseases works well and the health system is well equipped in this&nbsp;approach&nbsp;of reactive care.&nbsp;&nbsp;<\/p>\n\n\n\n<p>But why does this reactive approach need to change?&nbsp;&nbsp;<\/p>\n\n\n\n<p>The reactive approach to healthcare is both expensive, and, to some degree, ineffective in meeting the needs of&nbsp;today&#8217;s&nbsp;population, from the&nbsp;healthiest&nbsp;individual to the complex chronically ill.&nbsp;&nbsp;<\/p>\n\n\n\n<h4 class=\"wp-block-heading\">The burden of chronic disease&nbsp;<\/h4>\n\n\n\n<p>The\u00a0<a href=\"https:\/\/www.who.int\/\" target=\"_blank\" rel=\"noreferrer noopener nofollow\">World Health\u00a0Organization<\/a>\u00a0estimates that 50% of the global burden of disease is chronic illness. Chronic diseases are\u00a0long-lasting, having\u00a0compounding\u00a0and sustained effects\u00a0on the social,\u00a0physical,\u00a0psychological and economic\u00a0levels\u00a0of the individual and broader society.\u00a0\u00a0<\/p>\n\n\n\n<p>Chronic illnesses are mostly characterised by complex causes, many risk factors, long latency periods and result in functional impairment or disability. Most chronic illnesses do not fix themselves and are generally not cured completely.&nbsp;&nbsp;<\/p>\n\n\n\n<p>According to a\u00a0<a href=\"http:\/\/www3.weforum.org\/docs\/WEF_Harvard_HE_GlobalEconomicBurdenNonCommunicableDiseases_2011.pdf\" target=\"_blank\" rel=\"noreferrer noopener nofollow\">study<\/a>\u00a0done by\u00a0<a href=\"https:\/\/www.weforum.org\/\" target=\"_blank\" rel=\"noreferrer noopener nofollow\">World Economic Forum<\/a>\u00a0the global economic impact of the five leading chronic\u00a0diseases\u00a0&#8211;\u00a0cancer, diabetes, mental\u00a0illness,\u00a0heart\u00a0disease, and respiratory disease could reach\u00a0US$47 trillion\u00a0over\u00a0the\u00a0next 20 years.\u00a0\u00a0<\/p>\n\n\n\n<p>Is our current health system equipped for the future?&nbsp;<\/p>\n\n\n\n<h4 class=\"wp-block-heading\">A paradigm shift for modern medicine&nbsp;&nbsp;<\/h4>\n\n\n\n<p>Today, our world of healthcare is quickly transforming from a reactive to a proactive system.&nbsp;<\/p>\n\n\n\n<p>Having&nbsp;a better understanding of the underlying mechanisms and causes of&nbsp;Chronic&nbsp;Diseases&nbsp;as well as technological advancements is actively shaping contemporary medicine.&nbsp;&nbsp;&nbsp;<\/p>\n\n\n\n<p>The\u00a0<a href=\"https:\/\/www.ama-assn.org\/\" target=\"_blank\" rel=\"noreferrer noopener nofollow\">American Medical Association<\/a>\u00a0states that more than 98,000 annual deaths are preventable.\u00a0We\u00a0need to take a more active role in our\u00a0health and\u00a0make\u00a0healthy\u00a0lifestyle changes that support the\u00a0wellness\u00a0of our whole\u00a0being.\u00a0\u00a0<\/p>\n\n\n\n<p>The healthcare landscape is experiencing a tremendous paradigm shift to a new framework&nbsp;defined by the four&nbsp;Ps&nbsp;(P4) of&nbsp;medicine, a&nbsp;term coined by biologist Leroy Hood.&nbsp;&nbsp;<\/p>\n\n\n\n<p>The premise of P4 medicine is that in years to come medical practice will be transformed by biotechnology into a more targeted approach that manages a person\u2019s overall health as opposed to simply managing a patient\u2019s disease. Rather than the medicine of the past that worked on population averages, the medicine of the future will focus on the individual.&nbsp;<\/p>\n\n\n\n<h4 class=\"wp-block-heading\">&nbsp;The P4 approach to medicine&nbsp;<\/h4>\n\n\n\n<p>P4 is defined into four&nbsp;categories:&nbsp;Predictive&nbsp;Medicine, Preventative&nbsp;Medicine, Personalised&nbsp;Medicine&nbsp;and Participatory Medicine.&nbsp;&nbsp;<\/p>\n\n\n\n<p>These categories are set to modify the existing healthcare paradigm,&nbsp;elevate&nbsp;healthcare competency, reduce costs and help&nbsp;move our healthcare&nbsp;system&nbsp;from reactive to&nbsp;preventive&nbsp;and proactive.&nbsp;The active involvement of patients is necessary to guarantee effective self-management.&nbsp;<\/p>\n\n\n\n<h4 class=\"wp-block-heading\">Making the influx of information meaningful&nbsp;<\/h4>\n\n\n\n<p>The digitisation of medicine is making P4&nbsp;Medicine&nbsp;a reality today.&nbsp;New&nbsp;technologies are being created to handle, digitise, share and assess individual big&nbsp;datasets.&nbsp;&nbsp;<\/p>\n\n\n\n<p>The application of business intelligence and reporting to the modern dataset that makes up a patient&nbsp;is very powerful.&nbsp;Identifying&nbsp;patterns and revealing insights based on data can help clinicians&nbsp;make more informed decisions about a patient\u2019s care, but also empowers the patient to be actively involved in their&nbsp;health. The&nbsp;patient is not passively receiving care anymore, they are an active participant.&nbsp;<\/p>\n\n\n\n<p>The role of genomics is leading this&nbsp;health&nbsp;transformation. It allows scientists to evaluate an&nbsp;individual&#8217;s&nbsp;DNA and make predictions based on&nbsp;diseases&nbsp;the individual is genetically predisposed to.&nbsp;&nbsp;<\/p>\n\n\n\n<p>Modifying behavioural habits&nbsp;like avoiding smoking, maintaining a healthy diet, regular exercise and reducing alcohol consumption can improve health and reduce clinical problems.&nbsp;&nbsp;<\/p>\n\n\n\n<h4 class=\"wp-block-heading\">Can a&nbsp;health technology solution&nbsp;pave the path to&nbsp;proactive healthcare?&nbsp;<\/h4>\n\n\n\n<p>From including non-traditional data types, to enabling population stratification based on risk factors, a technology solution&nbsp;needs to&nbsp;embrace the flexibility to cater&nbsp;to&nbsp;an ever-growing longitudinal patient record.&nbsp;&nbsp;<\/p>\n\n\n\n<p>As the medical history definition expands to include genetic tests and social determinants of health, clinical decision support systems need to be put in place to help the clinician in navigating the amount of information available and making the best decision possible.&nbsp;&nbsp;<\/p>\n\n\n\n<p>Finally, the solution should carefully consider patient engagement.
