Patient information is often siloed in various healthcare facilities and clinicians have no ready means to access patient health information outside their facility. <\/p>\n\n\n\n

This often leaves a patient having to repeat laboratory tests, or at risk of medication errors. Patients need a digital care plan that outlines their treatment plan, adjusts to meet their changing needs, and follows them as they go through each step of their treatment. One Canadian province is well on its way to achieving exactly that through a Care Coordination Tool (CCT), which puts patients at the centre of their care.<\/p>\n\n\n\n

How the CCT works<\/h2>\n\n\n\n

The tool not only enables a living, digital\u00a0care plan that can be can be viewed and\u00a0updated by members of a patient\u2019s care\u00a0team, but it also allows authorized family\u00a0members to have access to pertinent\u00a0details of the plan and be apprised of their\u00a0family member\u2019s progress. <\/p>\n\n\n\n

Care plans are\u00a0made more transparent to both patients\u00a0and their families, which leads to a better\u00a0understanding of the specific procedures,\u00a0medications and desired outcomes. CCT\u00a0also allows for patients to have greater\u00a0control over their health, as they can review\u00a0their care plan and ask for adjustments as\u00a0appropriate.<\/p>\n\n\n\n

On the clinician side, CCT allows authorized healthcare providers to create and update care plans and give secure visibility to other providers within the patient\u2019s circle of care. It also offers users the ability to receive notifications which are sent whenever the care plan has been altered. CCT is already seeing promising results, with increased communication translating to better patient care and bridging of important care gaps.<\/p>\n\n\n\n

Putting care back on track <\/strong><\/h2>\n\n\n\n

Stories of patients in need of a solution like CCT abound within our healthcare system. One story described a patient who was dealing with two chronic diseases but was stuck bouncing between care providers, with few positive results. Even though this patient attended regular appointments the care providers often were unable to communicate with each other \u2013 leaving the high-needs patients feeling helpless and care providers unsure of next steps.<\/p>\n\n\n\n

By setting up patients with a coordinated\u00a0care plan and a care team that reflects\u00a0the needs of these patients, we can help\u00a0clinicians collaborate with ease. The CCT\u00a0enables meaningful access to patient data\u00a0for multiple care providers, and can provide\u00a0the most accurate patient data, both past\u00a0and present.<\/p>\n\n\n\n

The care team is also given\u00a0greater insight into the patient\u2019s treatment\u00a0plan and their progress. As a tool for care\u00a0providers, the CCT can be viewed, updated,\u00a0and facilitate ongoing communication\u00a0between members of a care team and those\u00a0they care for.<\/p>\n\n\n\n

Following the path<\/strong><\/h2>\n\n\n\n

But the CCT\u2019s strength doesn\u2019t just include\u00a0bridging communication gaps between\u00a0care providers, it also gives the patient\u00a0a voice. <\/p>\n\n\n\n

An example is one of a 90-year old\u00a0palliative patient that has multiple\u00a0specialists within his care team. One of\u00a0the aspects of his care that mattered most\u00a0to him was making sure that his family\u00a0understood his end-of-life wishes, primarily\u00a0the desire to stay in his home rather than be\u00a0constrained to a hospital bed. But as with\u00a0many cases, family members may not be\u00a0ready or willing to discuss these decisions\u00a0and difficult choices. Through the CCT,\u00a0end-of-life wishes can be captured on the\u00a0care plan where they are recorded and can\u00a0be shared with family members in order to\u00a0help make important decisions. <\/p>\n\n\n\n

Patients\u00a0appreciate the ability to relay their wishes\u00a0as it pertains to their care journey and\u00a0benefit from increased dialogue within their\u00a0circle of care, including family members.<\/p>\n\n\n\n

In\u00a0the end, the palliative patient was able to\u00a0have a peaceful death at home which was\u00a0conveyed by way of his care plan.\u00a0Patients shouldn\u2019t be expected to navigate\u00a0our healthcare system alone. With help\u00a0from the Care Coordination Tool and\u00a0other digital solutions, we can tear down\u00a0communication barriers and provide\u00a0patients with quality care that takes their\u00a0voices into consideration. The CCT gives\u00a0care providers a means of wrapping care\u00a0around the patient, and allows them to\u00a0build plans that evolve with patients as\u00a0they travel through their path of care. <\/p>\n\n\n\n
