Transforming care for chronic conditions in Northern Ireland

The Northern Ireland Electronic Care Record (NIECR) was rolled out across the whole country in 2013, to provide health and social care professionals with a single view of key patient information. Today, this has over 20,000 active users and has won a number of industry awards. In recent years, the solution has been extensively and consistently broadened in capability to support care.
In this session, Stephen Beattie, eHealth Programme Manager at Business Services Organisation will focus on two recent developments zoning in on chronic conditions, the Diabetes Care Pathway and Patient Portal Pathfinder. The Diabetes Care Pathway now facilitates integrated care all adult and paediatric diabetes patients across Northern Ireland. The Patient Portal aims to empower those living with dementia and their circles of care to take better ownership of their own health, manage the condition more effectively and engage with health care professionals.

Connecting Care in the South West: six years of innovation

The Connecting Care Programme first went live in 2013, with the ambition of facilitating a more integrated service to patients by better sharing information across the Bristol, North Somerset and South Gloucestershire region. In this session, Fran Draper will give practical advice on tackling information sharing at scale, including how to productively work with a wide range of partners across health and care. She will also cover some of the innovations that the project has made to move beyond the remit of a standard integrated digital care record, including document sharing using standards for interoperability, API management and inclusivity for mental health. Don’t miss this session that will demonstrate the benefits of integrated care, tips on how to achieve it and the pitfalls to avoid.
