{"id":7731,"date":"2018-12-19T22:58:55","date_gmt":"2018-12-19T22:58:55","guid":{"rendered":"http:\/\/orionhealth.weareiceberg.co\/?p=636"},"modified":"2023-03-15T21:00:51","modified_gmt":"2023-03-15T21:00:51","slug":"what-is-the-blue-button-2-0-api","status":"publish","type":"post","link":"https:\/\/orionhealth.com\/uk\/blog\/what-is-the-blue-button-2-0-api\/","title":{"rendered":"What is the Blue Button 2.0 API?"},"content":{"rendered":"<p><a href=\"https:\/\/bluebutton.cms.gov\/\" target=\"_blank\" rel=\"noreferrer noopener\" aria-label=\" (opens in a new tab)\">Blue Button 2.0<\/a>&nbsp;is an API from the Centres for Medicare &amp; Medicaid Services (<a rel=\"noreferrer noopener\" aria-label=\" (opens in a new tab)\" href=\"https:\/\/www.cms.gov\/\" target=\"_blank\">CMS<\/a>) that incorporates data for 53 million beneficiaries. It contains four years of data that reveals information about the health of each individual, including the Medicare plan they are on right through to the primary care and medicines they receive.<\/p>\n\n\n\n<p>It is an initiative in the U.S. to link claims data from Medicare and make this available to the consumers \u2013 i.e. the people who the claim was about. The Blue Button 2.0 API uses the&nbsp;<a href=\"https:\/\/www.hl7.org\/fhir\/\" target=\"_blank\" rel=\"noreferrer noopener\" aria-label=\" (opens in a new tab)\">HL7\u00ae&nbsp;FHIR\u00ae&nbsp;standard<\/a>&nbsp;as the format and&nbsp;<a rel=\"noreferrer noopener\" aria-label=\" (opens in a new tab)\" href=\"https:\/\/oauth.net\/2\/\" target=\"_blank\">OAuth 2.0 standard<\/a>&nbsp;and&nbsp;<a rel=\"noreferrer noopener\" aria-label=\" (opens in a new tab)\" href=\"http:\/\/hl7.org\/fhir\/smart-app-launch\/\" target=\"_blank\">SMART<\/a>&nbsp;as the authorisation mechanism.<\/p>\n\n\n\n<p>Developers can integrate with the Blue Button 2.0 API which will help individual patients and healthcare organisations to access and monitor health information in one place.<\/p>\n\n\n\n<h5 class=\"wp-block-heading\"><strong>How to connect to the Blue Button 2.0 API<\/strong><\/h5>\n\n\n\n<p>Let\u2019s look at how to connect to the Blue Button 2.0 API using the&nbsp;<a href=\"https:\/\/orionhealth.com\/uk\/blog\/what-is-smart-about-smart-on-fhir\/\">SMART client that we discussed in a previous blog<\/a>. There are 2 things we need to do:<\/p>\n\n\n\n<ul>\n<li>As an app developer, we need to register our app (in this case the SMART client) to Blue Button2.0 (BB2).<\/li>\n\n\n\n<li>As a user of the app, we need to register ourselves with BB2 so we can access our own data.<\/li>\n<\/ul>\n\n\n\n<p>In this blog, we\u2019ll only concern ourselves with registering the app, as there are test logins available. The first thing we need to do is to create a developer account on the BB2 sandbox&nbsp;<a href=\"https:\/\/sandbox.bluebutton.cms.gov\/v1\/accounts\/create\" target=\"_blank\" rel=\"noreferrer noopener\" aria-label=\" (opens in a new tab)\">here.<\/a><\/p>\n\n\n\n<p>Once you have an account, you\u2019ll need to register your application. There\u2019s a button on the site and you\u2019ll need to supply the following details:<\/p>\n\n\n\n<ul>\n<li>Name of the application \u2013 \u2018myBBapp\u2019 sounds elegant.<\/li>\n\n\n\n<li>Client type \u2013 choose \u2018confidential\u2019 \u2013 this means we can save a secret and it uses the \u2018authorisation code\u2019 grant type that SMART uses.<\/li>\n\n\n\n<li>Grant type \u2013 choose \u2018authorisation code\u2019.<\/li>\n\n\n\n<li>Redirect URIs \u2013 assuming you\u2019re using the SMART client. <\/li>\n<\/ul>\n\n\n\n<p>You can ignore the other fields then check the agreement to the service agreement and save. Once you\u2019ve done that, you get a number of elements provided, of which you\u2019re going to need a couple in particular:<\/p>\n\n\n\n<ul>\n<li>The<em>&nbsp;client id<\/em>&nbsp;\u2013 uniquely identifies the app (not the user of the app) to BB2 \u2013 technically the OAuth server in BB2.<\/li>\n\n\n\n<li><em>The secret<\/em>&nbsp;\u2013 because you said that you could keep one when you registered.<\/li>\n<\/ul>\n\n\n\n<p>Now you\u2019re ready to load these details into the SMART client.<\/p>\n\n\n\n<p>Load the client, click the \u2018Edit Server list\u2019 to the upper right and then click \u2018Add\u2019 above the list of servers to add a new one. Here\u2019s the screen you\u2019ll get:<\/p>\n\n\n\n<figure class=\"wp-block-image\"><img decoding=\"async\" src=\"https:\/\/orionhealth.weareiceberg.co\/wp-content\/uploads\/2019\/05\/picture1-1.png\" alt=\"\" class=\"wp-post-7731 wp-image-637\"\/><\/figure>\n\n\n\n<p>Fill in the field on the form using the values you entered and received from the BB2 site. Note that the baseUrl (It is mentioned in the developer site).<\/p>\n\n\n\n<p>It doesn\u2019t appear that the actual value of the scope matters, so I just left it at the default.<\/p>\n\n\n\n<p>Make sure you set the \u2018Where to open browser\u2019 to \u2018Separate tab\u2019. When you login, the SMART client app will open a separate tab to enter your login details rather than using the iframe in the main page. This is because of security restrictions made by Blue Button that prevents the login opening in an iframe \u2013 the original tab will remain open and you\u2019ll still see the progress comments there.<\/p>\n\n\n\n<p>Once that\u2019s done, save it and then you can select the server in the list and login. After retrieving the Capability Statement (which contains the authorisation endpoints) you\u2019ll need to click a \u2018continue\u2019 button (a browser security measure \u2013 it requires human intervention to authorise the opening of the separate tab).<\/p>\n\n\n\n<p>Assuming you\u2019ve entered everything OK, a new tab will open with the BB2 login form in it. At least in Chrome, the tab opens automatically \u2013 other browsers may require you to select it. This means that the app was properly authenticated, and it\u2019s time for the user to identify themselves. If this was operating against the real BB2 site (rather than the sandbox) then this would be the person wanting to access their own data. However, here we\u2019ll need a test login \u2013 there are a number of them available. From the&nbsp;<a href=\"https:\/\/bluebutton.cms.gov\/developers\/\">BB2 developer we<\/a><a href=\"https:\/\/bluebutton.cms.gov\/developers\/\" target=\"_blank\" rel=\"noreferrer noopener\" aria-label=\"b (opens in a new tab)\">b<\/a><a href=\"https:\/\/bluebutton.cms.gov\/developers\/\">site<\/a>.<\/p>\n\n\n\n<p><em>The first user is BBUser00000, with password PW00000!, and these sample users continue all the way to BBUser29999, with password PW29999!. (Note:&nbsp;<\/em>the !<em>&nbsp;at the end of the password is required.) Enter any one of these to continue.<\/em><\/p>\n\n\n\n<p>Next, you\u2019ll be presented with a form informing you that the application you are using (the SMART Client) wishes to access your personal data, and is that OK? To proceed you need to \u2018Allow\u2019 this. Assuming you do, you should land at the query screen. This means that everything has worked and you can now start issuing queries. If this were a real app, it would now be able to retrieve data and display it to the user in whatever format it wants.<\/p>\n\n\n\n<p>Before you do that, take a look at the \u2018Supported Resources\u2019 tab. You\u2019ll note that only a small number of resources is supported \u2013 just as it says in the documentation. Try entering a query like \u2018Coverage\u2019 \u2013 which asks for all coverage resource. You\u2019ll only get a few \u2013 just those that apply to the currently connected user. This is an example of the server applying its own business logic \u2013 only supplying data that the current user is entitled to.<\/p>\n\n\n\n<p>And a reminder that the query is an&nbsp;<a rel=\"noreferrer noopener\" aria-label=\" (opens in a new tab)\" href=\"http:\/\/hl7.org\/fhir\/search.html\" target=\"_blank\">FHIR search<\/a>. If you want a single resource, then you\u2019ll need to use the&nbsp;<a href=\"http:\/\/hl7.org\/fhir\/search.html#id\" target=\"_blank\" rel=\"noreferrer noopener\" aria-label=\" (opens in a new tab)\">_id query<\/a>. For example, the coverage resource has an element \u2018beneficiary\u2019 that is a reference to the Patient (in this case the same as the logged in user \u2013 it\u2019s a patient facing service remember). For example, suppose the beneficiary reference is:<\/p>\n\n\n\n<figure class=\"wp-block-image\"><img decoding=\"async\" src=\"https:\/\/orionhealth.weareiceberg.co\/wp-content\/uploads\/2019\/05\/screen-shot-2018-12-19-at-83205-am-1440x208.png\" alt=\"\" class=\"wp-post-7731 wp-image-638\"\/><\/figure>\n\n\n\n<p>To get the actual patient resource, use the query: Patient?_id= 19990000000001.
