SAN DIEGO — Qualcomm Incorporated (NASDAQ: QCOM) today announced that its subsidiary, Qualcomm Life, Inc., is collaborating with Orion Health, a leading population health management company, to develop a joint Chronic Care Management (CCM) Solution in response to the new Medicare CCM Reimbursement CPT codes.
Research has demonstrated that care management, such as medication reconciliation, coordination among all care providers, arrangements for social services, and remote patient monitoring reduces the cost of care for chronic disease patients while improving overall health. Now, as part of its growing emphasis on CCM, Medicare will reimburse providers for certain non-face-to-face care management services provided to Medicare beneficiaries covered under the traditional Medicare fee-for-service program.
