“KeyHIE aims to improve care coordination at all participating facilities in Pennsylvania,” wrote Monica. “And in an industry subject to as many technological shifts as healthcare, that requires the ability to adapt.”
In the article, Monica cited a number of engaging facts about KeyHIE’s innovative initiatives, including the HIE’s new Information Delivery Service (IDS), a tool designed to:
- Centralize an individual’s disparate health information
- Enhance population health management for various caregivers by identifying high-risk and chronic-condition patients, providing alerts and notifications, embedding incoming information directly into health records, delivering data to multiple providers “at once or separately,” and more
- Deliver notifications in accordance with the DirectTrust exchange framework
- Save money for the HIE’s participating practices. (According to KeyHIE, the tool is saving at least one practice some $56,000 a year)
Monica also described the HIE’s “Transform” tool, which empowers organizations to ingest data into the HIE—even if they have disparate systems and extract data from “minimum data sheets (MDS) or outcome and assessment information set (OASIS) forms”—and turn it into documents that comply with HL7.
