This session describes how the Dorset STP footprint is planning to deliver their 2020 Digital Vision. Including the Local Digital Roadmap and Dorset Care Record, the plan utilises learning from a wide-ranging clinical services review, and the implementation Better Together, a programme previously responsible for helping to integrate health and social care across the region.
Andy Hadley, Head of IT Development NHS Dorset CCG highlights how this vision has been incorporated within their Sustainability and Transformation Plan, in order to enable sustainable improvement in health and care outcomes through delivering: person-centred, outcome-focused, preventative and co-ordinated care.
